Job summary

The Integrated Discharge Team based at the Luton & Dunstable Hospital has an excellent reputation for the safe and effective management of complex discharges and plays a vital role in facilitating early and complex discharges, so people live or recover in the familiarity of home.

We are an enthusiastic 'can do' team who are very supportive to each other and enjoy the challenges of a complex discharge and the opportunity to test our knowledge and skills to ensure our patients receive the care they need on discharge from hospital.

In return we will actively encourage and support you in your development providing regular supervision, personal development plans, identify both formal and informal training needs for which we are able to take advantage of multidisciplinary training programmes.

Main duties of the job

As part of the Integrated Discharge Team, you will be responsible for providing a safe, effective and efficient discharge for patients as they return to the community from any point in their hospital pathway including End of Life and admission avoidance.

To assess patients and work with families and the multidisciplinary teams to ensure full understanding of the community options when organising packages of care., including assessment and prescription of nursing equipment.

To work in partnership with a range of multi-disciplinary and organisational personnel within both the hospital setting and community. To ensure safe discharge for End-of-life patients and those requiring Health Care Funding.

To facilitate clear and effective communication channels between patients, relatives, carers, and professional community colleagues to ensure seamless patient care and provide/receive sensitive patient medical information.

To enable patients and carers to participate in their discharge destination thus, encouraging independence and choice.

To clinically assess for equipment to keep the patient safe and enable staff to provide care in the Community setting.

To practice clinical and management leadership for staff within the integrated team and in regular 1:1 sessions with delegated members of the team.

To inform and resolve difficulties associated with cross boundary working in relation to complex assessments outcomes, patient needs and the various options available.

  • Nursing patients with a wide range of conditions including terminal illness and physical disabilities.
  • Experience of multi-disciplinary working.
  • Demonstrates understanding of safeguarding issues
  • Can demonstrate an ability to contribute towards a safe environment
  • Can demonstrate how s/he has promoted 'best practice'

  • Social Care Awareness
