Dudley Integrated Health and Care NHS Trust (DIHC) has developed and integrated primary care with community-based services within the Dudley healthcare system since 2020 with its core aim being community where possible and hospital when necessary. Our purpose is to improve the health and wellbeing of our local population in Dudley. We are currently working with organisations within the health care sector in Dudley and the Black Country System to look at the best way to continue to deliver the services to our patients. As such the services within the organisation may transfer to other organisations over the course of the next 12 months.

We are recruiting Advanced Clinical Practitioners (AHP's / HCPC or Nursing /NMC ) to work within the Sedgley, Coseley and Gornal PCN in Dudley. For more information about the role, please contact Chris Baron - Deputy Operations Manager

Main duties of the job

The post holder will provide care for patients within the Primary Care Network. Our current priorities are:

  • Improving Access to Care.
  • Improving Prescribing
  • Improving Cancer Diagnostic Pathways
  • Enhanced Health in Care Homes

You will work in one or multiple practices within the PCN. In each practice, you will have support from a named GP who will be available to review decisions when you feel this is required.

Advanced Clinical Practitioners (ACPs) are healthcare professionals, educated to Master's level or equivalent, with the skills and knowledge to allow them to expand their scope of practice to better meet the needs of the people they care for. ACPs are deployed across all healthcare settings and work at a level of advanced clinical practice that pulls together the four pillars of (1) clinical practice, (2) leadership & management, (3) education, and (4) research.

The Advanced Clinical Practitioner (ACP) will act autonomously within their professional scope of practice, providing care for patients face to face, telephone consultations, and home visits.

The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

The ACP will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
