Enablement Pathway -
This is an exciting opportunity for people who have lived experience to work within a dedicated,mental health, community enablement pathway, across secondary mental health care serviceswithin Coventry and Warwickshire.We are a team of nurses, support workers, occupational therapists, psychologists who are part of awell-established but expanding multidisciplinary team. The Enablement Team are using the ethos ofCHIME (Connections, Hope, Identity, Meaning and Empowerment0 to promote the work we do.
This opportunity will utilise the specialism of .................to work within a rehabilitative framework ofempowering and enabling, whilst maximising independence and adhering to a recovery focusedoutcome whether this be employment, training or sustained meaningful activity and routine.
The successful applicant work as part of a team that provides services to vulnerable people, somewith complex mental health needs and provide evidence based interventions working acrossboundaries of care, organisations and roles.
To build links with and liaise with external agencies to work together in a collaborative approach tomeet the needs of the person accessing services.
To share personal experiences of recovery and coping to build connection and provide support.
To ensure that the person accessing the service understands and has a clear pathway of care acrosssector/agency boundaries with key contact points/named staff.

- To ensure that the person accessing the service understands and has a clear pathway of care across sector/agency boundaries with key contact points/named staff.
- To work effectively as a member of a team to engage individuals in a number of activities and engagement with services.
- To support people accessing services to identify their recovery pathway and ways in which they can achieve their own goals.
- To support the team in implementing care planned interventions, including those supporting families and carers.
- To provide practical support to people in developing and managing dignity and independence.
- To support people accessing services with issues around benefits, housing and welfare rights.
- To provide support with daily living.
- To provide information on health promotion.
- To promote involvement within the service for the person accessing services and carer (friends and family).

- Relevant level 3 qualification in Health and Social care and/or relevant lived experience
