We are seeking a compassionate & experienced Mental Health Nurse to join our evolving team of placed based homeless workers. This unique & bespoke role, In partnership with CGL offers the opportunity to deliver trauma-informed, person-centered care to individuals facing homelessness, mental health challenges, & substance misuse.You will work closely with the Coventry Clinical Lead & CGL to bridge gaps in service provision & improve outcomes for this highly marginalized population who face significant health inequalities & barriers to care. This is a patient facing community role.

Main duties of the job

Key Responsibilities

Provide mental health support to adults who are rough sleeping, homeless, or at risk of homelessness.

Facilitate access to drug/alcohol services/support & mental health services.

Collaborate with voluntary & statutory services, including physical health & outreach teams.

Jointly assess & support individuals with dual diagnosis in partnership with CGL.

Work flexibly & autonomously, this may include on occasions outside st&ard hours, to meet client needs.

Advocating for service users, reducing disengagement, & helping them navigate services.

Providing responsive clinical input, including crisis management & harm reduction in community settings.

Actively contributing to multi-disciplinary team (MDT) meetings to ensure joined-up care.
