Job responsibilities
1. Provide holistic care for Service users (SU) with a Learning Disability and/or complex health needs this may include those with Autism.
2. To support enhanced packages of care in a variety of settings to
prevent and avoid inappropriate hospital admissions.
2. To be skilled in a wide range of specific clinical/behavioural Interventions, e.g., enteral feeding, oxygen therapy, oral clearance/suction, behaviour management and epilepsy management
3. Understand, support and evidence compliance on all care quality commission outcomes,
4. Act as named Nurse for allocated core group. Developing good relationships with the family and the wider MDT.
5. To assess, plan, implement and evaluate nursing and clinical care plans in collaboration with individuals, family and the wider MDT
6. Awareness of National and local policy and the implications for service delivery and own actions.
7. To administer medication in line with CWPT policy. Understand the pharmacology of medicines, use of as required medicines. Transcribe medication onto administration sheets and assist in writing of specific health protocols for as required drugs.
8. Carry out all relevant aspects of care mostly without direct supervision adhering to CWPT policy/procedure and NMC guidelines. Respond quickly and appropriately to constantly changing situations amidst frequent interruptions and distractions. The service may be Geographically isolated, therefore no immediate support.
9. To work in partnership with other agencies and organisations.
10. Ensure that dignity, choice, respect, independence, and the rights of the people that we support are maintained at all times.
11. Lead the team on duty, confident delegation to ensure that the service delivers high quality care.
12. Identifying and managing any health and safety issues, reporting to the Manager at the earliest convenience
13. To represent the service at internal and external meetings.
14. The post is multi role, therefore all aspects of duties will be carried out e.g., domestic cleaning, laundry, cooking, shopping, and gardening. SU/CYP are to be encouraged to participate where possible.
Communication
1. To be aware of the communication needs, both receptive and expressive of each individual and endeavour to meet those needs through programmes and recommendations identified in their care plan.
2. Form effective working relationships with departments within the Trust and external agencies.
3. To communicate effectively with parents/carers to ensure continuity of care. To offer advice/support/guidance/reassurance to families.
4. Help establish and maintain good communication within the service, ensuring continuity of information over a 24hour period is maintained across the whole staff team by providing detailed and through handovers.
5. Attend regular team meetings having an active contribution in a professional, open and honest way.
6. Participate in planning, developing, and implementing training programmes for the service.
7. Share appropriate information with staff from other departments and work alongside other organizations to ensure good teamwork.
8. Promote good relationships within the local community, with the general public and voluntary organisations
9. Adhere and contribute to Safeguarding Adults Policies and Procedures understanding the principles of DOLS and the Mental Capacity ACT
Analytical and Judgemental Skills/Freedom to Act
1. The post holder will most work without direct supervision and will be required to deal with a wide range of situations requiring analysis and clinical judgement or decision making.
2. Assist the manager in ensuring that aims and objectives are set for the service and reviewed on an ongoing basis.
3. Complete confidentiality is expected in all aspects to the job and adherence to NMC professional code of conduct, and the Trust policy.
4. Attend staff development programmes within the service and attend workshops, training courses, seminars and conferences as required and feed back to the staff team as appropriate.
5. To ensure that all complaints, incidents, untoward occurrences, or situations affecting the care provided to the service user are incident reported and reported to their line manager at the earliest opportunity.
6. To support the manager to ensure that the Trust Policies and procedures are embedded into the service, ensuring that staff are aware of and understand them.
7. There is a requirement to attend Statutory and mandatory training and any other training directed by the line manager.
8. Training in Safety Intervention to the required level deemed by the Service Level Agreement will need to be undertaken and updated annually.
9. Ensure Revalidation, Preceptorship, and Continued Professional Development plans are met through appraisals and regular 1-1 and clinical supervision.
Planning and Organisational Skills
1. Ability to effectively delegate to junior staff, managing difficult and challenging communication at times.
2. To have good time keeping and organisational skills.
3. To have a flexible approach often having to change priorities due to frequent interruptions and challenges.
4. To support with the management of the duty roster, highlighting any deficiencies and organising cover at short notice.
5. To plan and organise SU/CYP and MDT meetings, sometimes at short notice.
6. To support the planning of staff training.
7. To manage own outlook diary ensuring attendance at relevant meetings and sending apologies when needed.
Physical Skills
1. Attend annual safety intervention training to the required level deemed by individual risk assessment.
2. To be able to move at pace, competently support the moving and handling of individuals.
3. To support individuals with movement this may include pushing a wheelchair
Responsibility for service users
1. Enable to maintain personal contacts as appropriate
2. Responsible for the Health and Safety of all
3. Responsible for the Clinical assessment and formulation of individual care plans
4. Share information and involve parents/carers in the planning and implementations of care
5. Encourage feedback and discussion with the service user, their parents/carers regarding evaluation of care
6. Organize and conduct visits to Service User and their families to collate and update information
7. To undertake the role of Nurse in charge and plan and co-ordinate staff members daily, exercising professional accountability
8. Act as a Named Nurse for designated Service user
9. Implement care as planned in individual care plans
10. Recognise and respect the rights of the Service user and their parents/carers in the care you provide
11. Provide the necessary information to enable individuals to make informed choices in all aspects of care
12. Meet the needs of individuals cultural/religious beliefs recognising their personal preferences
13. Attend meetings, share information with the wider MDT that supports the assessment of need, planning of positive behavior support packages, care plans and risk assessments.
