Enablement Pathway -

This is an exciting opportunity for an experienced Clinical professionally qualified person to work within a dedicated, mental health, community enablement pathway within Coventry and Warwickshire, following substantial new investment. leading and managing our highly motivated and skilled team. The Team has been developed in the last 18 months.

We are as excited by this service development and the opportunity it provides to improve patient care, reduce the need for inpatient treatment and contribute to the development of innovative interventions. The expansion of our community services will provide clinicians with an opportunity to shape its development and evaluate new ways of working in the field; part of a well-established but expanding multidisciplinary team. Focussing on repatriation of patients in out of area rehab settings. Admission avoidance and discharge for those patients with complex and high intensity use of inpatient beds.

This will give you an opportunity to lead and manage the team and to work within a rehabilitative framework of empowering and enabling, whilst maximising independence and adhering to a recovery focused outcome whether this be employment, training or sustained meaningful activity and routine. Working collaboratively with our community providers and VCSE partners .

Main duties of the job

The post holder will be required to provide senior clinical/expert leadership to the service as well holding an operational management portfolio. They will be a critical part of the specialist, expert place-based MH and WB multi-agency team, providing individualised care for complex patients who required a higher level of care and treatment.

They will support the delivery of an effective, collaborative working environment across the neighbourhood so that all MH services in place, are adapted locally to meet the specific needs of that population and PCN. They will need to work in partnership with other Neighbourhood team leads encompassing matrix working, leadership and delivery of key targets and measurables for the defined population. In addition, approximately 40% of the post-holder's job plan will provide protectedclinical capacity and leadership to include clinical supervision of staff andcontribution to both place-based, and neighbourhood based clinical activities including planning and delivery of the multi-agency response to the needs of the most complex patients.They will be aligned to a specific neighbourhood (PCN(s) and will provide expert MH clinical leadership within the development of the multi-organisational MH&WB team within that setting. They will attend and lead the neighbourhood MH&WB MDTs and develop their local knowledge specific to the local population, suggesting futureinvestment and transformation opportunities to the Head of Place.

  • Registered professional relevant to the post with significant post-qualification experience (RMN, Social Worker, AHP)
  • Educated to a Masters level or demonstrable knowledge at the same level
  • Evidence of other leadership training and commitment to personal development

  • Ability to lead and manager successful teams.
  • Ability to understand complex data and information including written reports
