Our Coventry Community Neuro and Stroke Rehab Services (CINSS) are looking for a highly motivated Clinical Practitioner to join their interdisciplinary team.
We are excited to be able to offer this new opportunity for an enthusiastic RGN or HCP.
The service is a community based interdisciplinary therapy team, who support the timely discharge of patients from hospital. Our patients have ongoing rehabilitation or neuro management needs following a new or worsening presentation of a neurological condition, both stroke, and non-stroke.
The team work collaboratively with acute services, rehab and community networks and third sector services to deliver a patient-centred; goal focused approach to rehabilitation. Our team is an integral part of the stroke pathway, in reaching into both stroke and in-patient rehab settings.
This post will offer you the opportunity to consolidate and develop neurological and wider clinical skills. Your skills and experience will be integral to our quality improvement and service developments.
You will have the opportunity to shape and develop the role and embed it within an already successful integrated therapy offer. You will be working closely with established roles including Physiotherapists, Occupational Therapists, Psychologists, Speech and Language therapists and Therapy Assistants.
We will accept full and part time applicants (minimum 30 hours).
Main duties of the job
An inclusive team approach and excellent communication skills are essential. You will be expected to work independently and effectively within the Community Stroke and Integrated Neuro Rehab Service to ensure safe and effective management of these patients in the community. You will be able to prioritise your workload and time effectively to support an integrated approach.
You will be required to initiate effective history taking and undertake a medical clinical assessment to form a clinical diagnosis to support the implementation of a range of medical and care interventions for patients presenting with acute and chronic conditions who are receiving rehabilitation within the service.
You will also be required to be part of the Interdisciplinary team, supporting patients to meet their goals in a community setting.

Following a review of suitable providers to take forward the contract, the Coventry and Warwickshire Integrated Care Board (ICB) has confirmed its intention to award the contract for the provision of Adult Physical Community Services to University Hospitals Coventry & Warwickshire NHS Trust (UHCW). This change is anticipated to take effect from 1st July 2024.
Your application to work in Adult Physical Community Services will not be adversely impacted by the change in provider. Should you commence employment prior to the transfer, your contract of employment (that is your terms and conditions of service as at the point of transfer) will automatically transfer to the new organisation, under the "Transfer of Undertakings (Protection of Employment) Regulations 2006" (TUPE) as amended by the "Collective Redundancies and Transfer of Undertakings (Protection of Employment) (Amendment) Regulations 2014".
Should you commence employment after the transfer, your contract of employment will be issued by the new provider of Adult Physical Community Services.
