Physical Health Support Worker - Band 3

Full Time (37.5 hours a week 9-5)

South Warwickshire Community Mental Health Teams (Adults)

based at Leamington Spa / Stratford Healthcare.

We have an opportunity for an enthusiastic organised individual to join our dedicated multi-disciplinary community team. We aim to provide a quality service to individuals who are experiencing mental health problems across South Warwickshire.

We work with people and their families at home and support them in understanding their experiences and how to manage these. We also offer signposting to support with social needs, housing, benefits and role.

Main duties of the job

We are looking for an individual to support our physical health and healthy lifestyles work clinics. The role will involve booking, carrying out and documenting physical health checks in both clinic and home settings. Ideally you will have experience in taking bloods, performing ECGs, smoking cessation work, advising on healthy lifestyles and measuring height, weight, blood pressures. However, training on these can be provided.

You will form part of a large multidisciplinary team including doctors and nurses and you will have a key function in ensuring the physical wellbeing of those we service.
