Job summary

Planned Community Services and Discharge to Assess support patients in their home and play a vital role in helping patients to achieve their goals, following their hospital discharge and further along into their rehabilitation journey. We aim to instil self-management of long-term conditions in the community independently. You'll help to identify the support patients need and help them to achieve their goals and rediscover their passions. It requires you to think creatively, and tailor interventions for each individual patient. For some, it could be getting up the stairs unaided, going on a bike ride, or being able to cook their favourite dish! Every patient is different, and no two days are the same.
We have a permanent full time posts available in our Planned Community Services.
We are a dynamic service providing patient-centred assessment, treatment, and rehabilitation to residents of Coventry; aged 18 and over. Clinical caseload is varied and challenging including neurological, musculoskeletal, falls, multi-pathology, complex physical needs; including long term conditions and chronic pain.
Main duties of the job

You will be able to illustrate and reflect upon routine patient management and demonstrate an understanding of clinical reasoning. You will be able to prioritise your workload and time effectively to support appropriate caseload management and delegation.
There will also be opportunities for participating in various service development projects and attending regular in-service training.
At present the Planned Community Therapy service and Discharge to assess service operates from Monday to Friday core hours, however the job description reflects a seven-day service in case of changes to service needs.
