The Coventry and Warwickshire Partnership Trust [CWPT]provides comprehensive mental health/learning disability and community services to the population of Coventry and Warwickshire.
We are looking for a dynamic pharmacist to work within our psychiatric liaison service and local acute trust providers. Hosted by CWPT, this fixed term contract / secondment opportunity will focus on demonstrating the value of a clinical pharmacist supporting acute care for people with serious mental illness [SMI], within an acute physical hospital setting.
Main duties of the job
- offering advice to the acute hospital staff on medication-based issues such as initiation, switching or ceasing psychotropic medications
- liaising with the inpatient team on safe use of / managing the risks with clozapine in the acute hospital
- safe use of rapid tranquillisation
- Review of SMI patients and on-going psychotropic medication
- develop and test a rolling programme of training for pharmacy and nursing staff at the acute trust to highlight the risks associated with psychotropic medicines, and safe use of psychotropics
- support the acute team with risk recognition and prevention of adverse reactions, co-ordination of medication supply and monitoring arrangements with acute pharmacy teams, adherence to treatment guidelines and an enhanced patient experience.

- Confident and assertive when dealing with other professionals
