Part Time, fixed term 12 months contract

The Warwickshire Preschool Service is a multi disciplinary team working with individuals across the life span providing assessment, diagnosis and interventions for individuals with Neurodevelopmental Disorders such as Autism and Attention Deficit Hyper Activity Disorder (ADHD). We are passionate, innovative and dynamic teams who pride themselves on the quality of their work.

The service have recently received investment to manage the high numbers of children, young people and adults referred in and have procured external partners to work alongside CWPT to provide diagnostic assessments.

As part of this work the service need a capable patient flow Co-ordinator with exceptional organisational and time management skills and experience in supporting the work of a small team.

We are looking for a dynamic individual who has experience of NHS medical systems, has excellent IT skills and is confident in analysing data.

To support the Trusts move to remote working there will be an expectation for the successful applicant to work both from the office and also have to ability to work from home. Appropriate equipment will be provided.

The role of patient flow co-ordinator will be overseeing the flow of patient information to and from external partners. Ensuring there are appropriate and sufficient referrals being allocated to each provider and creating robust processes to manage and report on numbers. They will have responsibility for managing demand and capacity with support from the leads in the service.

The role will oversee the work of a number of a small team who are triaging and allocating cases to external partners including admin, clinical staff and a Data Quality Officer. They will also work closely with our information and contracting team to triangulate data and report on the activity of both CWPT and our partner organisations. The post holder will need to work autonomously, have a good understanding of our clinical records systems. They will need excellent communication skills to be able to liaise with our external partners.
