Job summary

Planned community services support patients in their home and play a vital role in helping patients to achieve their goals, long past their hospital discharge and manage their long-term conditions in the community independently. You'll help to identify the support patients need, and help them to achieve their goals and rediscover their passions. It It requires you to think creatively, and tailor interventions for each individual patient. For some, it could be getting up the stairs unaided, going on a bike ride, or being able to cook their favourite dish! Every patient is different, and no two days are the same.

We have a permanent part-time and full time post available in our Planned Therapy Services. Also available are Band 5 Physiotherapy posts progressing to a Band 6.

Main duties of the job

We are a dynamic service providing patient-centred assessment, treatment and rehabilitation to residents of Coventry; aged 18 and over. Clinical caseload is varied and challenging including neurological, musculoskeletal, falls, multi-pathology, complex physical needs; including long term conditions and chronic pain. Every patient is unique, and no two days are the same!

You will join our dynamic team of therapists who strive towards supporting holistic recovery and regaining independence supporting admission prevention, hospital discharge and supporting patients within the community.
