Job summary

The Clinical Review Team work with a diverse mix of clients including those who suffer with Mental Health, Learning Disabilities and those with Acquired Brain Injuries, so all level of registrations welcome.Working in the Clinical Review Team offers valuable experience when working with clients who are in funded packages of care. This work covers all parts of the patient journey in a funded package of care in the hope they would eventually live independently or with a small package of care in their own homes. You would need to undertake a programme of assessments in collaboration with other members of multi-professional teams where required. This will involve completion of the Reviewing Assessment document and potentially other clinical tools for example from the risk management or safeguarding toolkits.Working in this environment gives you unique opportunity to work our commissioners and understand how we can best serve our clients. Part of this would include bringing assessments to the clinical scrutiny panel and expand your plan, whether for repatriation or continued placement and justify the proposed care pathway. You will need to provide evidence of recovery focussed planning and an accurate understanding of current needs and level of risk and how these will be managed.

Main duties of the job

Act as an ambassador for the Clinical Review Team and the Trust, maintaining the highest standards of professionalism when liaising with service users and carers, placement providers, internal departments and external agenciesBe able to communicate complex and highly sensitive information where there may be barriers to understanding (eg. from service users or carers) and where negotiation skills are required
