Job summary
We currently have an exciting opportunity for dynamic, highly motivated, innovative and
forward-thinking Advanced Nurse Practitioner(ANP) to provide clinical expertise in the assessment,
development,planning, implementation and monitoring of patient care with the
community hosptials across County Durham . These Hospitals provide
rehabilitation, step down/up care and palliative and end of life services.
Priding ourselves with the multi-discinplinary approach for best patient
outcomes.
The post holder will require specialist knowledge to
demonstrate safe, competent clinical decision making and expert patient care
including diagnostic skills
Be a lead specialist in a defined area of nursing care.
Provides education and training to other staff, students
and/or patients/clients Undertakes research and leads clinical audits in own
specialist area
This post will be based within one of the community hospitals,
however there is a an expectation that the successful candidate will need to
travel between community hospitals within county Durham
Main duties of the job
Working alongside the Medical Practitioners
and the Advanced Clinical Practitioner
The post holder would be required to use clinical knowledge and experience
combined with sound clinical reasoning to interpret information obtained from
either patient, relative/carers or referring source in order to make clinical
decision, provide differential diagnosis and create records to support during
consultation period. Then in collaboration with senior decision maker agree
most appropriate outcome for the patient and expedite. As the ward ANP you will
review and be the prescriber of medications.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
