We have an exciting opportunity for a Band 4 Macmillan
Information and Support Cancer Care Coordinator to join our small and friendly
team. This exciting role will be based at Darlington Memorial Hospital and will
underpin the delivery of cancer information and support to people affected by
cancer.
We are looking for a highly motivated individual with excellent
communication skills and a passion for
high quality, person centred care.
You will support delivery of all elements of the Cancer
Personalised Care Agenda as well as providing guidance and supervision for the
centre volunteers, under the guidance of the Macmillan Information Services
Facilitator.
You will be qualified to NVQ Level 3 or equivalent and will
have experience working independently. You will also have good IT skills and
experience working in a healthcare setting. You will be able to demonstrate
resilience, compassion and an ability to work with people who may be distressed.

Main duties of the job
The post-holder will be based in the Macmillan Information
and Support Centre in Darlington Memorial Hospital where they will assist with
the day to day running of the service. This includes:
Dealing with enquiries from people affected by cancer, which
may be face to face, over the phone or by email.
Completing Holistic Needs Assessments and Personalised Care
and Support Plans.
Identifying patient's needs and signposting or referring to
relevant services.
Providing a listening ear to those who may be distressed.
Delivering Making Every Contact Count.
Assisting in the training and supervision of volunteers.
Facilitating the Help to Overcome Problems Effectively
Workshops (training will be provided).
Visiting hospital departments to offer support to patients
or family members as needed.
Completing centre administration tasks using Microsoft
Office packages (Word, Excel and PowerPoint).
Providing support to clinical teams such as putting together
information packs.
Maintain and update Social Media platforms.
Support the collection of patient experience data.
Provide administrative support for the Cancer Experts by
Experience Group.
Some travel across sites will be expected
