This
is an exciting opportunity to join a developing service within the Community
Directorate.
We are looking for an
enthusiastic and motivated registered nurse to join the Single Point of Access
team based in Seaham. The SPA Team is
the central access point for a number of community services working between the
hours of 8am and 9pm, 7 days a week. The team has an administrative structure
which is clinically supported by our nurses and therapist in the team. You will
triage referrals before ensuring that these are directed to the most
appropriate department. Referrals to SPA arrive in a variety of forms;
digitally, via telephone and through the ongoing work with partners in NEAS.
You will need to be proficient in prioritising workloads, responsive to the
needs of patients/families and be able to make informed decisions on the best
course of action in a fast paced environment.
You should have an excellent understanding of Community Nursing and
Urgent Community Response (UCR)
Once
fully competent in the role, SPA provide the flexibility to work from home on
an pre-agreed basis.
Main duties of the job
The
successful candidate will be responsible for delivering clinically safe triage
to the patients of CDDFT. You will also support and advise members of the team
who are dealing with incoming referrals and queries. Prioritising workload is
paramount in the role as you will be working between systems and dealing with
referrals depending on clinical need. You will possess excellent communication
skills and be able to work alongside a variety of clinical and non-clinical
roles. You should be a confident individual who is able to work as part of a
wider multidisciplinary team as well as alone.
Within
this role you will be working on a variety of systems including SystmOne,
HealthCall, digital telephone systems and NEAS systems. Your role is to safely
and effectively triage referrals to ensure that they reach the correct
destination depending on clinical presentation.
