term secondment opportunity offers an exciting opportunity for a suitably
qualified and experienced Nurse to work within the Specialist Coronary Heart
Disease & Heart Failure Team based at the Whitehouse in Easington. The post
covers core hours of 09:00-17:00 Monday-Friday with currently no weekend
working although that may change in the future
Main duties of the job
The successful applicant will be primarily caring for
patients with coronary heart disease (CHD) referred into the service following
acute events eg myocardial infarction and planned procedures eg elective PCI.
They will also be expected to support the Heart Failure Specialist Nurses
reviewing and caring for heart failure (HF) patients.
They will be directly involved in delivering cardiac
rehabilitation, to both CHD and HF patients, alongside the exercise
facilitators.
There may be an opportunity for professional development,
for example, attend courses relevant to CHD/HF.
Care is delivered in the patients own home and
in clinic / community venues around Easington
