We are looking for a dynamic individual to join the Intermediate Care Therapy Team at Chester LeStreet Community Hospital. The team works across the Durham, Chester-le-Street and Derwentside area providing therapy into rehabilitation beds within care homes and out into the community. The post involves working as part of multi-disciplinary team including Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners and Assistants.

Main duties of the job

The successful applicant will independently manage a caseload in the intermediate care beds and into the community providing holistic, short term, goal orientated interventions. Patients will present with a wide range of complex problems and will allow you to develop a highly specialist knowledge of many medical conditions and long term conditions and the application of the physiotherapy process with these patients.

The successful applicant would work Monday to Friday in the Intermediate Care Bed service. On occasions the successful applicant may also be required to work into the crisis response service, this is a six day service.
