This is an opportunity to join our established Adult
Physiotherapy team as part of the PulmonaryRehabilitation Service. This temporary post is open to new
graduates and established band 5physiotherapists.
Successful candidates will join our diverse team of
physiotherapists, assistant practitioners andphysiotherapy assistants to provide care and rehabilitation
to patients with a broad range of chronicrespiratory conditions.
You will work alongside colleagues to assess and treat
respiratory patients in an outpatient setting.
Working pattern will be Monday to Friday, there may be a
requirement to work weekends bankholidays or on-call.
You will be responsible for delivering evidence based
interventions to facilitate recovery, promote well-being and optimise independent function. The role includes
working autonomously and as part of awell established team. You will learn respiratory assessment
skills, contribute to individualised andgroup exerercise programs.

Main duties of the job
Under the superviusion of the band 7 clinmical lead (PR) you
will.
Assess, design, implement, and evaluate personalised
pulmonary rehabilitation exercise and education programs based on
evidence-based practices and patient-centered goals.
Monitor and assess patient progress towards goal
achievement, adapting exercise plans as necessary in collaboration with the
pulmonary rehabilitation multidisciplinary team.
Asist in managing waiting lists and facilitate appropriate
referrals to other services as needed.
Provide specialised guidance and education about the
pulmonary rehabilitation service to healthcare professionals across various
sectors, fostering effective collaboration and seamless patient care
coordination.
Produce comprehensive and concise reports for
general practitioners and other referring professionals, detailing assessment
findings and discharge outcomes post-completion of the Pulmonary Rehabilitation
program.
