An
exciting opportunity has arisen to work within the community fracture liasion service
and community Falls service. This role is for a associate practitioner with
skills in assessing and treating patients who are at a High risk of osteoprosis/Falls
. You will be carrying out an comprehensive assessment looking into the risk
factors for osteoprosis, falls prevention and analysing how to reduce the
patients future risks . Theses assessments may be carried out by telephone
assesment/home visits/ clinic and groups. As well as carrying out Falls
rehabilitation programmes with induviduals in the form of home visits or group
sessions.
Main duties of the job
You
will work as part of a team and at times you will be required to work
autonomously delivering support and interventions to our patients. You will
follow a specific plan of care implemented by the registered professional
focusing on a strength based approach to rehabilitation and sustained
improvement following the completion of a competency based training programmes.
You will be required to identify equipment provision required for patients and
carry out administrative and housekeeping tasks to support the service and
patient care. You will be expected to carrry out online Falls prevention
training sessions as well as exercise groups and telephone reviews.

Dependent on knowledge and experience you will be expected to contribute

management skills and participate in the monitoring and reviewing of team
performance.

to the promotion of an environment where health promotion and informed
patient choice is encouraged, developed and supported to its maximum potential,
within the available resources. An eagerness to learn and develop your skills
and knowledge by undertaking relevant training by attending mandatory training,
relevant training courses , use of eLearning and adhere to all trust policies
