Job summary
We
are looking for an Advanced Nurse Practitioner to join our established Community
Nursing Teams working in the North Durham Locality. We are looking for a
motivated and enthusiastic practitioner who is a Non-Medical Prescriber with
clinical skills who would welcome the challenge of working within a community
setting to provide support to the Community Nursing Team and GP practices to
provide high quality interventions to meet the needs of patients requiring
nursing and residential care in the community.
The
successful applicant will be required to work shifts over 7 days
Main duties of the job
The Advanced Nurse Practitoner ( ANP) will provide both planned and reactive
care including Urgent Community Response. The ANP will be integral to MDT's and
ward rounds in their aligned care homes and providing cover for their
colleagues

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
