An opportunity has arisen for a full time Discharge/Progress Facilitator within County Durham & Darlington NHS Foundation Trust

The purpose of the role is to facilitate the flow through the Acute Medical Unit, working closely with the team leader/coordinator to expedite discharges and safe transfers out of AMU

The successful applicant will be expected to work both as part of a team and to use their own initiative and work unsupervised to ensure the effective co-ordination of duties.

Main duties of the job

To promote the correct implementation of discharge processes throughout CDDFT to ensure safe and timely discharge and smooth transfer of patients

To liase closely with the ward coordinator to expedite early discharges.

Work closely with Social Services and Primary Care Trusts and other local authorities, private and voluntary organisations to ensure a seamless service, including working to reduce delayed discharges.

To link closely with other hospital sites to ensure effective use of beds.

To liaise with relevant staff and agencies on a daily basis to resolve daily operational issues and ensure discharges are managed effectively.

To understand and apply the skills and knowledge around complex needs and discharge planning, within a multidisciplinary team.

To chase up bed availability on a regular basis to ensure patients are transferred at the earliest opportunity

Job description

Job responsibilities

To work as part of the team under the direction of AMU coordinator, to ensure smooth flow of patients through the department.

To aid discharges from County Durham ad Darlington NHS Trust, smoothing the transition from the hospital back into the community, in line with Trust Policy by working with the multidisciplinary team (MDT), the patient and their support networks.

To identify issues which may affect the patients discharge home and work with the MDT and other agencies to resolve them.

To act as a central point of contact for discharge planning acting as resource for knowledge of the discharge planning processes, services available and how to access them.

To ensure referrals and formal notifications processed are followed in an appropriate and timely manner.

To be responsible for the prioritisation and action of cases using client centered principles to assist the multidisciplinary team and plan a smooth timely and effective discharge from hospital care.

To contribute to the planning, development, implementation and evaluation of discharge planning within County Durham & Darlington NHS Foundation Trust.

To work in partnership with relevant external agencies including CCGs, Social Services etc.

To liase with other departments and expediting clinical investigations the post holder will contribute to the effective and efficient running of the department resulting in significant reduction in patient journey times and improving performance against the 4 hour Emergency Department Standard.

To ensure tests are carried out at appropriate times escalating when standard operating procedures are breached.

The post holder will be instrumental in ensuring the discharge and patient transfer processes are efficient, timely and run smoothly including ensuring that Electronic Discharge Letters are sent to GPs where required

To manage a caseload of clients with multiple pathology and complex needs to facilitate timely and effective discharges.

To encourage completion of the Friends and Family test
