Job summary
The
post holder will act as a specialist resource in both Parkinsons disease and
related neurological movement disorders. This will encompass the nursing care
and management of people with these conditions, working closely with patients,
carers, Consultants and General Practitioners, therapists and social services.
The working environment will be in primary and secondary care settings across
trust sites. This is a Trustwide position, predominantly based in the North.
Main duties of the job
- The post holder will work as a key member of the medical and
multidisciplinary team, providing highly sensitive information, advice, care
management and support regarding patient care and treatment within the hospital
and the community.
- The post
holder will promote and initiate research based nursing and medical practice.
- The post
holder will identify and address individual needs, setting and implementing
programs of care.
- The post
holder will identify and address shortfalls in service provision Trust wide.
- The post
holder will initiate new specialist support developments within the hospital
and community settings Trust wide.
- The
post holder will be aware of the need to network across the specialty of
Parkinsons disease and related Movement Disorders, both locally and
nationally, developing links to improve service delivery.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
- Act in advisory role assessing physical, psychological and social
support needs of patients and carers.
- Provide accurate, research based
information including diagnosis, treatment, prognosis and disease management to
patients, family and career and other professionals
- Triage and
assess individual patients and take responsibility for assigning follow up.
- Act as a
resource for patients referred to Nurse led clinics by Consultant Geriatricians
and Neurologists.
- Attend
nurse led clinics trust wide and manage own case load to include telephone
clinics, home visits and advice and guidance.
- Provide
telephone advice to GPs regarding medication changes.
- Review and
titrate medication, depending on response and ensure that all parties are aware
of side effects, benefits and mode of action.
- Maintain
thorough and accurate records.
- Provide
information to patients about advanced therapies such as Apomorphine, DBS and
Duodopa.
- Act as a
patient advocate with family, employers to maximize independence.
- Develop
and implement training Trust and community wide to other professionals.
- Work
closely with charitable organisations and provide support and education to
local support groups.
- Act as
part of the team to deliver and assess patients having Apomorphine challenge.
- Communicate
highly sensitive information in a professional manner to patients, relatives
and all members of the MDT
