substantive Band 7 post offers an exciting opportunity for a suitably qualified
and experienced Nurse to work within the Community CHD & Heart Failure
Team. The post covers core hours of 09:00-17:00, Monday to Friday although
weekend working may be introduced in the future
Main duties of the job
To
provide clinical nursing leadership and act as a professional role model for
all members of the cardiology team.
To manage
individual patients needs, including symptom assessment, physical examination,
requesting investigation and prescribing as appropriate and within agreed
pathways of care.
Provide
clinical expert advice and support to patients, carers and the wider
multi-professional teams involved in the management of patients.
Ensure
effective communication throughout the patient journey.
Explores
complex issues and makes complex decisions encouraging the effective
participation of everyone involved
Non-medical
prescribing in line with legislation
Developing
therapeutic interventions to improve service user outcomes
To line
manage designated staff members within the service where required
Acts as a
role model providing mentorship and/or clinical supervision to other
staff/learners supporting them in applying theory to practice
Supporting
others to develop knowledge and skills and encourages promotion of
learning/creation of learning environment. Developing service user/carer
education materials and acts as a resource for improving health

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
