Job summary
The Targeted lung health Check is a new programme designed
specifically to target the population of County Durham who may be eligible for
CT imaging based on evidence based criteria. This is a national programme
looking at identifying early cancers and other incidential findings such as
benign nodules, COPD/Emphysema, Coronary artery disease and potential Aortic
Aneurysms . The postholder will be required to manage a caseload offering
virtual consultations to patients identified as eligble via primary care. There
will also be a requirement for data collection and entry.
This is a predominately home based role with
minimal requirement to travel across site however this may be required as the
role develops.
Main duties of the job
To organise work schedules for yourself and the team (day to
day, week to week, month to month-Flexibility required)
Run nurse led telephone clinics assessing patients and
escalating any concerns to the team lead
Provide specialist nursing advice
Work autonomously as we all as part of a
multi-disciplinary team.
Ability to provide health promotion and smoking cessation
advice.
Formulation of individualised care pathways with the
patient and the carer
Communicate with clinicians as necessary.
Contribute to and support the work of colleagues in the
Multi-Disciplinary Team
Support teaching of the multi-disciplinary team to maintain
the learning environment.
Demonstrate responsibility for professional development
using Personal development Plan Portfolio.
Incorporate health education for health promotion in
working practices.
Demonstrate practical application of specialist knowledge.
Identify, collect and participate in the evaluation of
information to support multi-disciplinary clinical decision making.
Contribute to dissemination and implementation
of evidence based care in the writing of shared guidelines /protocols for care.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
