Job summary

East community Rehabilitation Team are a small and dynamic therapy team providing community physio and occupational therapy to adults with physical health issues within the Easington Locality. The team also works collaboratively with the community nursing services working over a 6 day rota providing same day response for patients with nursing/ rehab and care needs that require a 2 hour crisis respons. Domicillary experience desired but not essential.

As part of ongoing development within the team we are looking for a dynamic Band 5 Physiotherapist to join us serving both the intermediate care and domicillary service.

You will have the opportunity to be involved with service development as part of the wider community therapy services and an opportunity to develop your clinical and leadership skills within the supportive environment of the east therapy team.

Main duties of the job

To work as a member of the Teams around practices (TAPS/HUBs) covering the East Locality

-The post holder will independently manage a caseload and provide assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes)

-On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients.

-The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care.

-The post holder will work under the guidance of a senior therapist

To undertake a comprehensive assessment of patients, using investigative and analytical skills, to formulate individualised treatment plans, using clinical reasoning skills, and utilising a wide range of treatment skills. Not directly supervised, but seek advice as and when required.

As an autonomous practitioner, to be professionally and legally responsible and accountable for all aspects of your own clinical and non-clinical activity associated with individual patient care.

To delegate treatment tasks to unqualified staff inclusive of assistants and care support workers.

To plan and organise clinical activity, with appropriate time scheduling in order to complete all clinical and administrative duties including statistical record keeping.

To be responsible for the correct and safe use of the physical environment including equipment and to be responsible for the allocation of these resources.

To be responsible for clinical record-keeping in accordance with the guidelines for documentation and legal requirements.

To contribute to the supervision of undergraduate Therapists where placements within speciality areas have been designated as appropriate.
