Job summary
To
provide a comprehensive and integrated specialist nursing service for people living
with diabetes both in the Hospital and in the Community. The post holder will join other colleagues
providing a service for adult people with diabetes. They will act as an
additional resource for nursing colleagues and other health workers offering
expert care, advice, teaching and support and building up communication links
as needed. Flexibility is essential in this post.
Main duties of the job
Main duties include providing an
inpatient diabetes service. Assessing and reviewing acutely unwell patients to
enable they have a positive patient journey.
Ensuring that they are receiving
high quality diabetic care as well as ensuring that their medications are
appropriate and patient centred to achieve good outcomes and reduce long term
effects of diabetes.
Supporting ward staff to adopt
care plans specific to the individual and providing ongoing education.
Supporting patients with the
available technology eg CGM, Flash glucose monitoring and insulin pumps.
Empowering patients to take
control of their diabetes and promote self management.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

To develop
