Job summary
An autonomous practitioner with advanced
clinical skills and be a non-medical prescriber or possess one of these skill
sets and working toward the other. You will undertake a leadership role in the
assessment, co-ordination, case management and delivery of patient centered
care to a client group who present multiple and often complex health and social
care needs requiring the input of a range of agencies. You will work alongside
sisters, staff nurses, support workers and therapies within the community. You
will contribute to our Enhanced Health in Care Homes services (GP led) to
ensure care home residents are supported to make the best decisions about their
health needs and our care home providers and their staff are enabled to deliver
the highest standards of care. There is an emphasis on proactive and reactive
care delivery/planning.
Main duties of the job
A
senior clinical role that provides an opportunity to make a real difference to
patients' lives? This post is for an Advanced Nurse Practitioner (formerly Community
Specialist Practitioner for Long Term Conditions) who will need to be passionate,
innovative and motivated to join a well-established community team.
To assess patients, to
develop, plan, implement and monitor care. To provide highly specialist advice.
Maintains appropriate records.
Lead specialist in a
defined area of nursing care.
Provides specialist
education and training to other staff, students and/or patients/clients
Undertakes research and leads clinical audits in own specialist area
Takes a lead role in
assessment, diagnosis and treatment of caseload of patients/clients.
Undertakes a medical
history, completes a detailed physical and nursing examination, formulates a
diagnosis, initiates investigations and/or treatments and makes onward
referrals as appropriate.
Is an accountable
practitioner.
Maintains records as
autonomous practitioner.
Highly skilled at assessing
and managing risk.
Accepts the responsibility
for decisions made and actions taken.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
