Job summary

An exciting opportunity has arisen for a Band 3 Team Support Officer to join our friendly and forward thinking team within Community Services covering the Sedgefield Locality.

Main duties of the job

The team support officer is a key function in the management and day to day functionality of community nursing teams and the patients they care for. The purpose of this role is to work with and support the team with their clinical administration functions associated with the management of patients: accepting incoming tasks and referrals to the team, setting appropriate care plans. Using the autoplanner functionality within SystmOne to allocate the following days visits. Supporting the duty Sister to review allocated visits and supporting any changes required to the visit lists.

To be eligible for Health and Care Worker visa sponsorship you will usually need to be paid at least £25,600 per year

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
