Job summary

We are currently looking for a Health Care Assistants to join our Macmillan care team and Community Nursing team. This is an exciting opportunity for somone wishing to join our community team services.

Main duties of the job

The role involves providing holistic nursing care to patients in their own homes primarily. The sucessful candidate will be supported with any additional training to fulfil their role which will involve: wound care, skin care, venepuncture, catheter care, administration of insulin and palliative care and support alongside the team.

To be eligible for Health and Care Worker visa sponsorship you will usually need to be paid at least £25,600 per year

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Essential

  • Ability to communicate clearly and effectively
  • Able to work as part of team
  • Able to observe and record physical and social changes in patients
  • Able to identify when to report to a registered nurse/therapist
  • Able to work alone with patients within an agreed care plan
  • Able to work with minimal supervision
  • An appreciation of dealing with people in their own homes
  • Demonstrate the ability (with training if necessary) to undertake basic clinical activities such as venepuncture, simple wound dressings, blood glucose monitoring (this list is not exhaustive) and other clinical duties identified by registered nurse
  • Able to maintain confidentiality
  • Understand of working with other agencies/multi disciplinary working

Desirable

  • Able to use own initiative appropriately
  • Training list identified is not exhaustive and amendments can be make dependent on individual needs and profession as identified at appraisal

  • Ability for independent travel to fulfil requirements of the post

Qualifications

  • Minimum of 4 GCSE level C, or equivalent, including English Language
  • NVQ Level 3 in relevant area, e.g diagnostic and Therapeutic Care or Care or working towards or equivalent experience
  • Knowledge of Microsoft packages

Desirable

  • An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing

Experience

Essential

  • Experience of working within multidisciplinary teams
  • Experience of working with adults
  • Recent and relevant band 2 assistant experience within rehabilitation in a related clinical specialism within the NHS
