Job summary

An exciting opportunity has arisen for a Band 7 Team Lead to join our friendly and forward thinking team within Community Services covering the Willington, Crook and Weardale Locality, based in Crook.

This would suit a highly motivated and skilled Senior Nurse with the passion to drive forward high quality care.

The Team Lead role is to provide clinical leadership to community staff within the multidisciplinary team.

To undertake clinical role as an expert practitioner and active provider of specialist clinical care, maintaining clinical skills and demonstrating an effective role model for the nursing staff and other professionals with in the multidisciplinary team.

To ensure patient safety and clinical effectiveness, providing expert evidence based clinical advice and support to health and social care staff within the locality.

To coordinate and monitor the delivery of high quality nursing care that, where possible, meets identified patient need, supports the aims and objectives of the integrated multidisciplinary team, and remains within the boundaries of available resource.

Main duties of the job

The successful candidate will be able to demonstrate excellent communication skills, team working skills and will be dedicated to improving the patients' experience. You will need to demonstrate continuing professional development and a strong commitment to develop others and work closely within a multidisciplinary team. Ensure delivery of safe, high quality care in line with the organisations strategic vision. As a clinical lead, you will be expected to support the Locality Matron in driving the team to maintain a patient focused positive experience and environment. To undertake clinical role as an expert practitioner and active provider of specialist clinical care, maintaining clinical skills and demonstrating an effective role model for the nursing staff and other professionals with in the multidisciplinary team.
