Advanced Clinical Practitioner - Clinical Night Team

Job summary

The Clinical Night Team (CNT) are a team of established Advanced Clinical Practitioners (ACP), the Qualified ACPs provide advanced clinical cover 19:30 -07:30 across all adult inpatient areas and the emergency department for patients with a decision to admit and awaiting a ward bed. They will arrange appropriate management and treatment plans for in patients, supporting admission areas with the assessment of patients with a variety of undifferentiated presentations whilst working within the trusts guidelines, policy and procedures. The CNT work alongside the on-call medical and surgical teams supporting the medical staff to remain in the admission areas to clerk new admissions. Provide the AKI service over night. The ACPs are supported by a team of Assistant Practitioners.

Main duties of the job

The successful candidate will work within the Clinical Night team independently reviewing and treating patients across all divisions in the organisation.

You will work alongside both resident and senior doctors actingas an autonomous practitioner delivering a high quality service. Responsibilities will include assessments of patients using a range of assessment methods to determine ongoing plans of care.

You will also be responsible for independent reviews of inpatients who are receiving ongoing ward based care and escalate care where escalation is indicated to the relevant speciality and medical staff.

Essential

  • MSc in Advanced Nursing Practice, Clinical Practice or evidence of appropriate level 7 study in a relevant specialist clinical field.
  • Professional clinical qualification.
  • Independent/Supplementary Non-medical Prescribing (where legally able to).
  • Advanced Life Support qualification.

Desirable

  • Leadership/Management qualification

Knowledge & Experience

Essential

  • Highly motivated and experienced practitioner with substantive range of experience within relevant area eg emergency / general medicine
  • Broad & advanced knowledge of clinical pathophysiology
  • Sound level of knowledge in relation to invasive and non-invasive diagnostic and therapeutic procedures
  • Experience in managing chronic and acute health issues
  • Proven leadership experience including delivering to targets and service improvement.
  • Ability to manage conflicting pressures to maintain streamline service delivery
  • Demonstrates current knowledge of professional and NHS issues and the implication for clinical environment.
  • Experience of clinical research
  • Evidence of leading on clinical audit and translation to service development
  • Advanced clinical assessment and diagnostic skills based on interpretation of imaging and pathology results
