Job summary
The Deputy Chief Medical Officer for Primary and Community Care Integration is a key member of the Trust's senior medical leadership team with several discrete areas of responsibility including the medical lead for community service transformation and primary care integration with community and secondary care. In addition, further areas of responsibility will be agreed with the Chief Medical Officer as required.
The post holder has, in addition, a responsibility to deputise for the Chief Medical Officer in contributing to the corporate management of the Trust and deputise at Trust Board meetings as required.
This is a part time role (6PAs) and is open both to existing consultants at Lancashire Teaching Hospitals and external applicants.
Main duties of the job
The post-holder will be the medical lead for community service transformation and primary care integration with community and secondary care working closely with colleagues within the Trust and wider system to develop improved pathways of care for patients, reducing reliance on hospital admission. While the operational responsibility and accountability for patient care rests with the Clinical Directors/Operational Managers this post holder will need to ensure:
- Support development of effective pathways of care for patients with long term conditions that reduce reliance on hospital admissions alongside divisional, community and primary care colleagues.
- Support development of Virtual Wards, frailty and SDEC services alongside divisional leadership teams.
- Support the development of clinical strategies within the Trust and across the Place Based Partnership to align with the NHS 10-year plan.
- Work with stakeholders to contribute to wider leadership development plans across the organisation.
- Secure effective engagement with clinicians at all levels and encourage cross professional collaboration to deliver service improvement.
