What's the Role About?

In this unique role, you'll track patient pathways from the referral through to their first stage of treatment, analysing large amounts of data, each day to ensure every step is accurately reported onto our Patient Tracking Lists. Your attention to detail will ensure that all specialties we monitor have the critical information they need to meet national standards. It's a fast-paced, rewarding role where your work will have an impact on patient satisfaction and care.

Main duties of the job

Some of the key duties of the role would include, performing daily validation of the 18 Weeks/RTT PTL (Patient Tracking List) database to ensure accuracy and compliance with RTT standards. Completing detailed Validation & Error reports, identifying and rectifying errors as necessary. Validating patient letters and documents across multiple specialties, ensuring all information is accurate and up-to-date. To provide an accurate monthly representation of RTT standards to the Trust. Communicating confidently and professionally with internal and external staff members, ensuring smooth communication across teams. You will be carrying out personal workstreams, ensuring all agreed timescales and goals are met. Participating actively in team meetings, contributing ideas. Fostering a positive and open team culture is key, in alignment with the Trust's values, encouraging development, growth, and collaborative working.

Job responsibilities

Duties to include but not be limited to:

  • Assist with improvement and developments in the timely capture of sound and accurate data across the Trust.
  • To complete daily validation of 18 Weeks/RTT PTL database, update QCPR and identify and escalate risks to compliance.
  • To validate 18 weeks breaches for admitted and non-admitted patients and update QCPR accordingly.
  • Extract pathway information from clinical correspondence in shared electronic folders and on ALMA, Evolve, and Bluespiers or in paper notes.
  • Use of appropriate computer packages including Bi Portal, Prism, GI Tool, to extract data for analytical purposes.
  • To escalate inaccuracies, inconsistencies and gaps in RTT data collection.
  • Action validation/exception reports as appropriate ensuring pathway information is obtained, corrected and issues escalated as required.
  • To ensure all information relating to the patient RTT pathway is communicated effectively and by the most appropriate medium
  • Actively promote the importance of the patients right to access services within the maximum wait times and support the understanding of the current 18 weeks RTT measures and standards.
  • Escalate to senior staff all risks to potential breaches of RTT pathways identified as a result of analysis.
  • Assist in the collection of data for audit purposes and support management teams to identify areas that require pathway improvements.
  • Assist in ensuring that the working practices within the team effectively support achievement of the 18 week RTT measures and standards.
  • Fulfil above functions by the efficient use of computerised systems, email and manual systems.
  • Audit the effectiveness of procedural documentation to ensure practice matches procedure.
  • Continual evaluation of the effectiveness of own work streams and their agreed timescales.
  • Participate in planned rotation between sites as necessary.
  • Take an active role in Team Meetings as required.
