Job summary

This is an exciting opportunity for a Registered Mental Health Nurse/Social Worker/Occupational Therapist to join our specialist service for people experiencing a first episode of psychosis as a Care Co-ordinator.

You will be joining a client centred, dynamic and friendly multi-disciplinary team, who prides themselves on delivering high quality, evidence-based interventions for those experiencing first episode psychosis.

The purpose of the role is to reduce the stigma associated with psychosis, provide an early intervention to reduce the length of time that people remain undiagnosed and untreated and to offer support and guidance throughout the persons recovery journey.

Main duties of the job

Under line management and clinical supervision from within EIT, the post holder will maintain and co-ordinate timely and appropriate interventions, for a caseload of service users living in the Bolton area. The post holder will work within the framework of CPA, and in line with relevant clinical pathways as outlined within the EIT Operational Procedure.

To hold and manage a caseload of service users who have been identified as experiencing their first episode of psychosis, including care planning and delivering psychosocial interventions. To work as part of our integrated MDT, and to participate in a clinical decision making process to determine the most appropriate management plans for all service users who are under the care of the team. To undertake and record comprehensive risk assessment, devise and implement risk assessment plans for service users at risk of vulnerability, self harm, self neglect, or risk of physical, sexual, emotional harm to others, using the CRASP risk assessment tool. To participate in the duty/assessment rota operated by the Early Intervention Team.
