Community Inclusion Worker

Job summary

The post holder will work as part of the Community Inclusion Service as a senior support worker under the supervision of the team manager and Occupational Therapist. The main purpose of the role is to work with service users, both on a one to one and group basis, in a goal and recovery focussed way, to enable individuals to improve their wellbeing and move towards independence. This will involve helping service users to overcome barriers to sustaining engagement in their activities of choice and supporting service users to access mainstream activities in the wider community. The post holder will be responsible for developing links in the community, providing one to one support to enable service users to be active, develop their skills and contribute to their communities.

The post involves the co-creation of goal plans with service users and regular reviews and monitoring of service user progress.

The post holder will work with service users who experience severe mental health problems and assist them to access employment, education and training (vocational) opportunities.

Main duties of the job

  • Support for people with severe and enduring mental health to move towards and maintain independent living within communities and to be placed on a path to work
  • Person centred and recovery focused approach - Holistic support that considers all aspects of a person's recovery.
  • Assessment and goal planning, clear expectation around move on, support plans subject to regular reviews
  • Provide a range and levels of support responding to need e.g. intensive to less intensive one to one, looking at social and community links and next steps
  • Tenancy support - assist people to live or move onto more independent living and ensure the support and resources are in place to give the tenancy the best chance of success
  • Flexible one to one, group work, and peer support across the city - Community development role linking in with Community based services and peer led groups/activity.
  • Pathway to volunteering, training and employment - linking with Working Well and other Mental Health employment programmes.
  • Evidence based monitoring of service user progress.
  • Provide step down from care co-ordination - secondary services, reducing the need for ongoing or high-cost care package
  • Provide interventions, opportunities and support in line with service users identified needs; this involves both 1-to-1 support and group work

  • NVQ Level 3 in a Health and Social Care setting or equivalent knowledge and experience.
