Senior Practitioner Patient Flow (Discharge Schemes)

Job summary

Promote effective patient flow based on individual service users care pathways whilst ensuring least restrictive practice and optimizing lengths of stay and support the GMMH Trust wide goal to reduce out of area placements.

To promote and facilitate timely admissions and discharges within inpatient unit, step-down and crisis beds through safe, timely and effective bed management as an integral part of the CPA process. To operate as a member of Inpatient MDT to provide clinical expertise to facilitate safe discharge from the inpatient service, stepdown and crisis beds.

To liaise closely and effectively with key staff in other agencies CMHT, HBTT, EIT, Later Life, LD, Rehabilitation Services, Brokerage Team, step-down, crisis bed and housing providers etc. to maximize safe and timely discharges.

To act as a resource to provide education, advice and support relating to all aspects of admissions and discharge planning and provide supervision and training as required to junior members and students of the GMMH Trust Wide Patient Flow Service.

Participate in relevant GMMH Trust Wide Patient meetings and provide cover as required for the Hub & Spoke teams.

The post holder will take on specific responsibilities for the effective use of step-down and crisis beds and the Personalisation Fund, including appropriate use of hotels, deep cleans etc. to support safe and timely discharges.

Main duties of the job

o Triage all referrals to step-down and crisis beds commissioned as part of the Discharge Fund schemes to ensure all relevant details are correct, shared with providers to enable an appropriate decision regarding access to the beds.o Provide clinical expertise regarding admission and discharge criteria and ensure bed management policies and procedures are adhered to.o Ensuring the least restrictive options are applied at all times to manage service users, complete required documentation and update service users electronic clinical records as required.o Chair meetings with all step-down and crisis bed providers to review each service user placed there and agree actions to support moves on from these temporary beds.o Liaise with and advise step-down and crisis bed providers on any issues which arise during the service user's stay.o Liaise with CMHTs, HBTT, housing and VCSE providers to support service users and facilitate their moving onto more permanent accommodation. o Work with referrers to ascertain any alternative to the requested admission.o To take a lead on the appropriate use of the Personalisation Fund to prevent avoidable admissions, expedite discharges from inpatient, step-down and crisis beds.o To agree transport arrangements including number of escorts required as necessary for service user transfers to and from the inpatient units.
