Job summary
This is an exciting position which will focus on the physical wellbeing of our service users with mental health needs. The post holder will be responsible for the leading on the physical health care agenda within the Central East Community Mental Health Team under the supervision of the Team Manager and the senior practitioners on the team.
The post holder will ensure the delivery of all physical health care Trust key performance indicators. They will act as a specialist resource and advisor to care coordinators within the community teams. The post holder will work across community settings, including clinic bases and home visits.
The post holder will be required to work, for most of the time, as an autonomous clinical practitioner for patient groups across a range of clinical settings. The post holder will be expected to forge strong links with the Care Coordinators, Physical Health Support staff, Primary Care services particularly GP practices.
Main duties of the job
Ensure PHIT assessments and interventions are completed for service users identified in high-risk groups in line with associated key performance indicators, e.g. CQUIN targets, etc
Undertake any audits that are related to physical health care e.g. Physical health improvement tools, blood sugar monitoring equipment and resuscitation audits and ensure that any actions are completed in a timely manner
To ensure that any physical healthcare risk assessments are completed in a timely manner and reviewed as appropriate
Liaise with medical staff as and when required
To ensure that patients have access to health improvement interventions and to signpost as appropriate
Participate in the functioning of the clozapine clinic on a weekly basis.
