Senior Practioner Patient Flow

Job summary

Promote effective patient flow based on individual service users care pathways whilst ensuring least restrictive practice and optimizing lengths of stay and support the GMMH Trust wide goal to reduce out of area placements.

To promote and facilitate timely admissions and discharges within inpatient unit through safe, timely and effective bed management as an integral part of the CPA process. To operate as a member of Inpatient Multi-disciplinary team to provide clinical expertise to facilitate safe discharge from the inpatient service.

To liaise closely and effectively with key staff in other agencies RAID, CMHT, HBTT, EIT, Later Life, LD and Rehabilitation Services etc to maximize safe and timely discharges

To act as a resource to provide education, advice and support relating to all aspects of admissions and discharge planning and provide supervision and training as required to junior members and students of the GMMH Trust Wide Patient Flow Service. Participate in relevant GMMH Trust Wide Patient Flow meetings and provide cover as required for the Hub & Spoke teams.Hub team work shifts to provide cover 24/7. Spoke team work predominantly 9am to 5pm with some out of hours and nights as required by service.The post holder will chair the local bed management meetings and board rounds and keep accurate records.

The post holder will participate in the local Bronze on call rotas as required.

Main duties of the job

Provide clinical expertise regarding admission and discharge criteria and ensure bed management policies and procedures are adhered to.

Gatekeep all referrals to the inpatient services ensuring the least restrictive options are applied at all times to manage service users, complete required documentation and update service users records as required.

Work with referrers to ascertain any alternative to the requested admission

To agree transport arrangements including number of escorts required as necessary for transfers to and from the inpatient units.

Maintain an up to date working knowledge of Mental Health Legislation, CPA procedure, crisis care planning, safeguarding and risk management.

Ensure KPI's, and relevant CQUIN and or CQC targets are completed and maintained.

Undertake clinical assessments as necessary, including patients placed in other hospitals

Undertake risk assessment and risk management to an exemplary standard to promote patient safety in line with Trust Policy and Procedure

Maintain timely and accurate clinical records including inputting data into clinical information systems and local recording systems.

Participate in supervision, appraisal and training required to maintain clinical competence and professional registration as required.

Engage in Team meetings

Engage in daily huddle meetings ensuring that demand and capacity is communicated effectively
