An opportunity has arisen for an experienced senior support worker to join a vibrant mental health inreach team within Health and Justice based at HMP Manchester. The post holder will work within the MHIT and will be an integral part of the wider clinical MDT.
This is an exciting opportunity to be a part of an established service within the Health & Justice directorate of Greater Manchester Mental Health NHS Foundation Trust. You will be supported with the security checking procedure (Counter Terrorism Clearance) to enable you to work in a High Secure Environment. It is essential that throughout your employment that you are compliant at all times with the necessary renewal checks every 5 years.
Main duties of the job
The post holder will assess physiological observations, including blood monitoring and ECG's, and promote healthy lifestyles for our service users. The post holder will assist in enabling service users to access the support they need to recover from their mental health and physical health problems, whilst in a custodial setting. They will also assist care co-coordinators in working across boundaries of care to plan, implement and evaluate care within care pathways as part of the stepped care model.
