Duty Worker

Job summary

Please note this a secondment for 6 months. It is open to all staff who hold a professional qualification ie. RMN, MH SW, MH OT

An opportunity has arisen for a highly motivated and experienced Duty Worker to join a vibrant integrated health and social care Community Mental Health Team.

In North Trafford CMHT the duty desk is permanently managed by two duty practitioners, this creates a streamlined and effective way of working. Referrals are dealt with safely and assessments are completed in a timely manner.

This post would provide the suitable candidate the opportunity to work within a highly skilled clinical team in the community, applying clinical assessment skills to support the care and treatment needs of vulnerable adults with mental health needs.

The age range of service users is from 16-70 yrs across the whole of the Trafford borough for those experiencing mental health difficulties.

Trafford has a varied socio economic group and is a multicultural area.

The successful candidate will screen and triage referrals to the team, conducting holistic and evidenced based Bio Psycho social assessments and contributing towards the MDT discussions and care planning.

Risk assessment and risk management is an essential aspect of this role and in addition to the expectation to participate in providing quality first line psychosocial assessments, the role also requires working proactively in forging professional working relationships with multi-agencies.

Main duties of the job

Carrying out initial assessments and responding to all new referrals received by the team. If referrals are not suitable to facilitate passing referral onto appropriate service.

Completing robust and holistic risk assessments which accurately indicate the needs of vulnerable adults .

Liaising with multiple agencies, third parties and patients, making onwards referrals

Advocating on behalf of vulnerable adults to achieve positive outcome sin their treatment and acre

Writing reports in a timely manner.

Contributing towards weekly MDT discussion.
