Secondary Mental Health Team

Job summary

The secondary mental health team at HMP Forest Bank are looking for a Band 5 registered mental health nurse who has completed their preceptorship to join our Health and Justice team. This post is a duty nurse role to support the wider secondary mental health team.

To work with the prison service partners in providing appropriate support and risk assessments for patients who are subject to prison service risk management procedures such as ACCT and segregation procedures.

To enter information on inphase system relating to safety and security of records of all interventions.

Main duties of the job

The service is a Monday to Friday 9-5

The role consists of answering clinical duty telephone calls, health system tasks and e-mails. Following up clinical information with community mental health team courts police mental health liaison and diversion teams.

Attending weekly SPOA meeting to discuss referrals and allocations.

Case management of complex cases. Triage of cases in a fast paced environment and follow up with community services.

To complete assessments on behalf of the wider secondary mental health team. Good documentation skills. To be clinically inquisitive to form good clinical formulation and risk assessment.

To support hospital referrals assessment and follow up.

To attend reflective practice monthly and drop in sessions when required.

Weekly depot clinic- documentation re booking monitoring of EPSE'S

To support the psychiatrist/advanced nurse practitioner clinics.
