Senior Operational Community Patient Flow Lead

Job summary

The successful applicant will support community teams in Manchester with a cohort of service users that are in an inpatient setting to transfer safely and timely back to a community setting.

They will work closely with the inpatient teams, brokerage and panel colleagues to create solutions for timely discharge out of an inpatient setting once they become clinically ready for discharge.

They will carry a cohort of complex patients requiring creative solutions to support them back to a home environment, supported accommodation, residential placements, nursing home and rehab settings.

The postholder will support junior staff in the CMHT teams and EIS with assessing service users capacity, arranging best interest meetings and documentation for packages of care. They will upskill staff, arrange training days and be a point of expertise to support staff to grow and develop the skills required to support patients out of a hospital environment.

Main duties of the job

  • Attend high level meetings with external colleagues
  • Unblock discharge barriers with inpatient teams
  • Take a complex caseload to support flow
  • To be the point of contact for community staff for advice and support and to upskill and train staff with processes to support people back to the community
  • Develop positive relationships with ICB, local authority and brokerage services to support the wider system
  • To provide strong and effective clinical leadership and line management for Manchester community teams promoting a positive and therapeutic culture that empowers staff to achieve optimal performance.

  • Pro actively utilises own judgement to provide options and resolutions which may involve complex facts. Optomising all professional advice from the MDT, family, carers to drive a way forward for example: case conferences, vulnerable adult panels, guardianship applications
  • Oversee the daily delivery of discharges including complex cases and patients clinically ready for discharge. Retain daily oversight of discharge progress with live and accurate updates to the Head of operations, and service managers.
