Job summary
Prescott House Community Mental Health Team are looking to recruit a motivated and enthusiastic Support Worker to join our successful, friendly and supportive team.
We are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties within the Salford area.
We are looking for a motivated individual who is flexible, creative, committed and able to adapt positively to change. You need to be organised, able to prioritise and work well under pressure.
Whilst this post requires no professional registration, experience within mental health is essential. The post holder's professional development will be supported by the service through regular supervision and training opportunities. Staff well being is at the heart of our CMHT and staff will be supported by peers and engage with team building events ,reflective practice groups, training and acupuncture.
Regular clinical supervision and training opportunities will be provided to the successful candidate.
Main duties of the job
The post holder will be a key member of the CMHT, working directly with service users under the care of the CMHT, as part of the Multi Disciplinary Team which provides care, treatment and support to people with a functional mental illness.
The post holder will provide support to those service users in line with the clinical pathways within the CMHT - namely Assertive Outreach and Dual Diagnosis pathway, as outlined within the CMHT operational procedure. The role also involves short term periods of care to assist service users to develop independent living skills.
The post holder will provide support, advice and practical assistance to an allocated group of service users under the direction and supervision of a qualified member of the staff team, following a care plan agreed with the service user to facilitate goal planning, achievement and recovery.
The post holder will support carers of service users and will provide a carers clinic where they will undertake carers assessments.
