Job summary

We are looking for an enthusiastic, motivated individual to work as a senior nurse practitioner and to assist with the day-to-day management of this innovative Salford Living Well Team. In this role you will provide both clinical leadership and line management supervision to staff within the service. Clinical time will include direct contact with people accessing the service, and deputising on behalf of the team manager when required. The senior nurse practitioner will also assist with future developments of the service in line with the NHS community mental health transformation framework.

Salford Living Well Service will enable timely access to mental health services when people need it most. The service will provide support to people with moderate, longstanding mental health difficulties, but this may also include support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team will be multidisciplinary and will also include statutory and non-statutory organizations.

The service currently operates between 9-5, Monday - Friday excluding bank holidays.

Main duties of the job

  • Facilitate the effective assessment, care planning and review of clients referred to Living Well.
  • Provide support for those people who present with the most complex needs both by conducting assessments & reviews.

  • To support GPs with managing risk and issues relating to medication, and to liaise with psychiatry colleagues about medication advice and diagnostic advice as required.

  • Support the MDT with reviews of complex cases.

  • In conjunction with the Team Manager and other Senior Practitioners undertake responsibility for the continuous improvement of the service provided by Living Well.

  • In conjunction with the Operational Manager /Team Manager identify opportunities for local service development and manage and lead the process of change.

  • In conjunction with the Operational/Team Manager identify unmet needs to support service planning and development.
