Job summary

An exciting opportunity has arisen for a part time Band 4 Resettlement Support Worker to join the Later Life Service supporting vulnerable Older Adults with housing issues.

The successful candidate will be based in North Manchester however the post is Citywide and covers all of the Manchester area which includes South and Central Manchester covering a diverse population.

The Resettlement worker will work alongside community and ward based staff in supporting service users with mental health difficulties with any housing related issues and in setting up in new accommodation. They will be working with people who are on the wards and whose discharge has been delayed due to lack of appropriate accommodation as well as working with community service users whose tenancies are at risk, or where their housing is having a negative impact on their health and wellbeing.

We are looking for a motivated and enthusiastic individual to join our friendly team.

Main duties of the job

To work alongside ward staff where a discharge from the ward is delayed due to accommodation issues

To work alongside community teams where service users accommodation issues are causing difficulties with their mental health or their tenancies at risk

Liaising with statutory and voluntary organizations

To make referrals to appropriate services

To support service users in setting up in new accommodation and helping with establishing utilities

Accompanying service user to appointments in support of any accommodation issues

Participate in Duty rota
