Home Based Treatment Team Support Worker

Job summary

The post holder will work as part of the South Manchester Home Based Treatment Team, to undertake allocated duties within the team, you will work alongside mental health practitioners, supporting patients, families and carers in their own home and also supporting patients who are on the community waiters and supported discharge pathway from the wards ensuring the highest possible standard of care in accordance with the policies and procedures.

The post holder will provide Support and Time to an allocated group of service users and promote their Recovery. They will assist the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans.

The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations to promote social inclusion and promote active participation in an individualised recovery plan using the agreed set of Team values and skills to underpin their day-to-day work.

Service users will have individualised care plans.

Main duties of the job

To demonstrate high-level communication

To utilise highly developed interpersonal skills including specific skills to facilitate communication when there are barriers to understanding e.g. sensory or cognitive impairment or in a hostile, antagonistic or highly emotive situation.

To establish and maintain robust communication networks with clients, carers, and other health care workers as well as external agencies.

To be an active member of the multi-disciplinary team (MDT)

To encourage and support clients to collaborate in their treatment programme and recovery.

To encourage self-help as much as possible, promoting the maintenance of skills and independence in clients

To support health promotion and act in a consultative role/advisor to other community services/agencies and personnel in relation to individual clients and their mental health issues.

To undertake planned interventions and feedback outcomes to MDT
