Job summary

The post holder will be employed by Greater Manchester Mental Health NHS Foundation Trust to provide 1 session to the Living Well Manchester service. The post holder will be working alongside a Consultant Psychiatrist who we are hoping to recruit into the service also. The LWM works with people who are not meeting the threshold for secondary care mental health teams, or who have declined/not responded to primary care mental health interventions either due to levels of complexity or uncertainty regarding their clinical presentation/additional needs

Main duties of the job

The target group of people for the LWM is adults and older adults (functional) who:

  • Have complex mental health and social needs.
  • May or may not have a mental health diagnosis.
  • Require a multi-agency approach.
  • Cannot be fully supported by primary care (including Talking Therapies for Anxiety and Depression).
  • Do not require a more intensive/specialist offer (e.g., CMHT or inpatient services)
  • Do not require access to urgent care/crisis services.

Key Tasks

  • To provide a comprehensive mental health assessment, this will involve taking a full psychiatric history and mental state examination.
  • To develop a management plan to address the mental health needs which may include medication and signposting to other services.
  • To provide and comprehensive risk assessment and risk management plan.
  • To record all information on the electronic patient record system.
  • To advise regarding any medical.
  • To provide follow-up appointments to monitor health and risk.
  • To identify any Physical Health needs that may require further investigation from patients own GP.
  • For those patients with complex needs and high risk to liaise directly with secondary mental health services in Manchester.
  • To discharge patients when no further intervention is required.
