Care Coordinator (Nurse, OT or Social Worker)

Job summary

Due to a promotion, we have had a vacancy arise in our otherwise-fully staffed team. You will be one of 12 care coordinators, working in an experienced MDT with a shared goal of providing the best possible care and support to our small caseload of service users. You will be working with service users who are experiencing a First Episode of Psychosis with a collaborative approach from the MDT to support their recovery.

Main duties of the job

The role involves managing a caseload of service users alongside our team of doctors, psychological therapists, occupational therapists, support workers and other professionals, to help them manage and navigate a first episode of psychosis.

You will be their main contact for the team and will meet with them regularly over their 2-3 year pathway with the team, making sure that all NICE guidelines for psychosis are adhered to and people receive a full package of evidence-based care.
