Job summary

Central Later Life CMHT are looking to recruit a Band 6 Community Mental Health Nurse to join a busy Multi Disciplinary Team. The post offers the opportunity to work within the CMHT and the Memory Assessment/Treatment Service. Full training will be provided around the Memory Assessment pathway.

There is the opportunity for professional development within this role.

Regular Management and Clinical Supervision is provided.

Main duties of the job

An opportunity has arisen for a qualified and experienced individual to join a busy and vibrant Community Mental Health Team for Later Life based in Central Manchester.The successful candidates will be required to fulfil the role of a CPN as part of a multi-disciplinary team; the care co-ordinator role is an essential component of the role. The team operates from Chorlton House, 70 Manchester Rd, Chorlton-Cum- Hardy, Central Manchester.You will be responsible for the management of a case load of clients with complex functional and organic mental health issues and associated social care issues, for which risk assessment and risk management is an essential requirement. In addition you will also assess people with memory problems and offer support following diagnosis. You will also participate in the Duty rota
